Strategic Provider Partnerships
YRN works closely with CARF-accredited, DBH-certified, and licensed treatment providers to ensure individuals are  serviced by reputable organizations that deliver evidence-based care. These partnerships allow us to streamline access and reduce wait times for critical services.

Ongoing Support & Follow-Up

Connection is only the beginning. YRN provides ongoing follow-up to:

Monitor attendance and engagement
Reconnect individuals who may disengage from services
Adjust linkage plans as needs evolve

Appointment Scheduling & Care Coordination

We take the next step beyond referral by:

Scheduling initial and follow-up appointments directly with providers
Coordinating intake assessments and required documentation
Communicating with provider teams to ensure continuity of care
Tracking engagement to support successful connection and follow-through

Transportation Support

Understanding that access barriers can disrupt recovery, YRN assists with:

Arranging transportation through Medicaid Managed Care Organizations (MCOs) and community partners
Coordinating rides for assessments, treatment sessions, and critical appointments
Problem-solving transportation barriers to ensure consistent attendance

Warm Hand-Off Approach
Our team utilizes a “warm hand-off” model, meaning we remain actively involved during transitions between services. We ensure individuals are not navigating systems alone, providing support until they are successfully engaged in care.

Barrier Reduction & Advocacy

YRN advocates on behalf of individuals to remove obstacles to care, including:

Insurance verification and navigation support
Assistance with documentation requirements
Coordination with probation, courts, or other systems when applicable
Linking individuals to additional supports such as housing, employment, and peer services
