- Enhance coordination for co-occurring disorders, with 70% of individuals engaged in integrated care within 30 days
- Promote housing stability, aiming to connect 60–75% of individuals to housing resources within 30 days
- Support successful housing placement, with a goal of 50–65% achieving stable housing within 60–90 days
- Expand access to essential resources, ensuring 85% of participants are connected to at least one critical support service
- Strengthen continuity of care, with 70% of individuals maintaining engagement across providers for at least 60 days
- Provide rapid crisis response, targeting 90% of urgent cases connected to stabilization services within 24–72 hours
- Increase economic stability, with 40–55% of participants connected to employment or vocational resources within 90 days
- Reduce barriers to care, assisting 80% of participants in resolving access-related challenges
- Deliver individualized recovery navigation, with 100% of participants receiving a personalized service plan
